Hello.  I have a 2 1/2 year old child that has been diagnosed with asd by a Neurologist and a psychologist from the University of Michigan autism center.  I signed a form-2 for the autism consultant to come out at his school and asses him so he can be certified.  I've been through this process before so I know it can be very draining.  The consultant told me to go home and make a list of why autism is effecting my son's education.  So I made a list off all of Broderick's issues.  I didn't state the issue and why.  For example I said Broderick has high anxiety when the routine changes.  Is that good enough?  Or should I say Broderick has high anxiety and will result in disruptions of the class?  I really need to  know if I'm wording this list right.  

I think you're wording is right, but maybe try not to say he would be distruptive to the class.  You can say he has high anxiety when his routine changes which may effect his learning because he acts out in such in such of way.

I think that the way you are listing them is the best way. You could list examples of how you've seen his issues affect him at his current school, at home, in the community, etc after each thing, but don't project what issues it could cause at school.

Example:
Broderick has high anxiety when the routine changes
-When at the grocery store if I skip a certain section of the store that we typically go through, (produce or meat) he will begin to cry and yell.
-When we've had snow days and Broderick hasn't gone to school like he was supposed to, he will perseverate on it the entire day, and can become aggressive because he doesn't understand why he's missing school.


...use examples of things that have already happened, that could potentially affect him in a similar manner at school.
