Also also what are your / your child's dreams...this is always a good start... one of the leading experts ( I think it was Judith Snow) in the field of transition planning  and self advocacy would describe transiton planning as planning for an enviable life...transition plans should address all areas of adult living-- social experiences, job/career, self care ( if necessary) and independent living/transportation.  I thnk NICHY has lots of articles on this
