Transition Plans

Hi everyone, as some of you may know I am a special education teacher.  I am working on my level II credential in California.  I'm taking a class about Transition Planning.  I have to admit I don't know anything about transition plans, as I teach 1-3 graders and my own dd is only in 2nd grade.  I need to write a paper on a problem within the transition process in special education.  I was hoping one of you with older kids (16 in California) has been through the transition planning process and could share an area of concern you have or had.
thanks!
drmomtojoe,
you must have read my mind.  I just found a wonderful website National Center on Secondary Education and Transition.  I have narrowed down my topic to either failure of IEP to create self-determination in students or making sure students and parents understand the differences between supports available to students for postsecondary school.

Emma, my ds is also not of transition age but I do have a transition background...

you could  look at involving students in their IEP...this is part of IDEA but in reality the student voice and parent voice are not truly heard...many students lack the skills necessary to participate in this meeting because they have not been taught how to participate and lack self-determination.   (Go to NICHCY and check out the info on student involvement in the IEP and self determination).

Great, good luck with your paper.
