Autism Reinforcer Checklist | Autism PDD

Anyone know where to find a copy of the Autism Reinforcer Checklist by William Jensen of the University of Utah (1997, revised)?

It's 10 pages long and includes such categories as edible, material, social, activities, domestic and token reinforcement.

The list contains check off boxes and 4 columns to rank reinforcers from least to most preferred.

If you know where it can be found online or sent to me as a PDF, that would be great!
