Take him to a pediatric optometrist for an eye exam. Kids should have this at age 3 anyway. The doc will put drops in his eyes and examine them, physically, and will also have eye exams that a 3 yo has some hope of cooperating with. So often we forget to check the obvious. I have a friend whose son is grown now. Turned out that, although he is extremely high functioning, he has some degree of Asperger's. When he was 3, he kept going up to all men who even faintly resembled his father and calling them Daddy. My girlfriend had his eyes examined (she had no idea at the time that he had AS) and it turned out that he is legally blind.
There is a saying that if you hear hooves thundering, it's likely it's a stampede of horses, not a stampede of zebras. We parents of kids with autism need to remember that they are kids first and the explanation for an issue may be something that affects typical kids, too.
Did the little girl resemble Sarah? If so, would it be possible that your son was trying to say that the girl looks like Sarah, but for example didn't know exactly how to communicate that?
My son has trouble recognizing people sometimes. A couple times he has gotten hung up on details (for example a change in hairstyle or new sweater) and not recognized his own teachers.
Other times he thinks he recognizes someone, again probably because he doesn't see the "whole picture" and thinks "dark hair plus brown skin plus girl equals my cousin".

i find my kids know people by there smell ,it is just has individual as a face is
so dont worry it will come in time.
try some picture cards of a man a woman a boy and girl and expalin them to him.

My 3 year old ds has two playmates he sees alot - sarah and jack.
He sees them quite often as they are our neighbors. We were at the park and he saw a little girl and called her Sarah. Im wondering if this is a sign as I read kids on the spectrum may have problems with facial recognition..also whenever he sees an old lady he says Grammy...

I actually think that's perfectly normal. My son is on the spectrum and never had that problem.
There are alot of more important signs that are noticed first.
