This type of info. is sooo invaluable, especially when you're cluless in your first IEP meeting like I was 4 months ago! It really gives parents a chance to participate when they have their own information. I'll be saving this for sure.I copied the sections that I needed and modified it a bit. I found it to be so helpful!
