Since school is almost out and my son will be attending another school for 6th grade, i would like to modify the current IEP. Do i ask for an IEP meeting at his current school or do I wait and ask for an IEP meeting at the new school? I would like to address locker situation, PE, transitioning between classes, etc.

Request one now. You should have a transition IEP when going from one school to another. We have one set for Adam in May. It is with his current spec ed teacher, and principal, and the spec ed teachers and vice principal at the middle school.

Ideally, you will have a transition meeting with the "old team" and the "new team" (or at least some members of each) sitting together at the table to discuss your son's needs. This is ideal because there is so much that cannot be conveyed on paper. Then the new team can talk with you about the IEP and either write a new one right now with service providers changing in the fall or write a new one in the fall based on the meeting that you have now.
