After filling the whole thing out and getting it set to mail in, I noticed that apparently it can be filled out and submitted online.  If you go to the site I posted and scroll through to the bottom of the last page, there is a website listed.  I haven't looked at the site, but perhaps you could submit the form that way.  Good luck!

By the way, I filled the form out but I question how accurate it'll be for us.  I noticed after I was finished that the form is meant for children 3-5 y/o (or if your children are older, for their behaviors from 3 to 5).  Luke is only 2!  Some of the language questions really didn't apply to him.  They might rate him lower than he is. :/  Or perhaps they'll see from his birthdate that he's only 2 and tell me that their results aren't quite fitting, due to his age.  Will let you all know what I find out, too!

I filled this out and sent it in several years ago. I think I got the results back 2 days later! I was stunned at the quick turnaround. It's a very good checklist in my opinion.Has anyone else done this?  Apparently the Autism Research Institute will 'grade' the checklist for you and send it back free in order to clarify the diagnosis of children who have been or may be diagnosed with autistic or "PDD" or 'autistic-like.'  Apparently, Dr. Rimland (who founded ARI) is extremely against using the term PDD.  I read an article he wrote on th subject.  Anyway, here is the link for the checklist if anyone is interested:

I agree that it might not be as accurate for a child under 3. So many
of the behaviors are a matter of age. I don't know that I would have
accepted any diagnosis of my son at that age.

know what happens with it.I have tried and tried to print the crazy thing out, for months, and it won't print on my printer! I am going to try again right now, since you've reminded me about it, and you guys think it is great!
